CREDIT CARDS:
*Credit Card or Debit payments are possible with advanced arrangements. Payment needs to be made by the time of the session please.
Please Note: Credit card fee will be added- 1.6% to 3.6% plus .25 cent fee per transaction (depending on which card is used) as charged by the charge card Merchant provider
INSURANCE:
**My office will submit for Workman’s Compensation, OUT OF NETWORK INSURANCE and MED-PAY AUTO INSURANCE, for auto accidents. *We are also a Denver Options Traumatic Brain Injury Service Provider (State of Colorado TBI Fund) and will bill them directly for your approved services with authorization.
GIFT CERTIFICATES:
Gift certificates are available in any amount. Please contact me and I will arrange a certificate in the name of the person receiving the gift.
We are otherwise happy to provide you with a receipt so that you can seek all other insurance reimbursement; we will provide you with a diagnosis and treatment code.
